What types of services does Partners in Care provide?

- Some Time for You - A program to assist and provide relief to caregivers.
- A Time for Transitions - Assistance for families that have experienced a recent loss.

There is a four-hour minimum per day for home health aide service.

What steps are involved in implementing services?
The process is very easy. For service in the five boroughs of New York City, Nassau County or Westchester County simply call 1-212-609-7700. Working closely with patient, family and physician, a Partners in Care registered nurse will recommend a plan of care. This plan is tailored to meet the patient's specific needs. Because no two patients are alike, we are careful to match our caregivers with the personalities and needs of the patient. A registered nurse will supervise your case.

What languages do the aides speak?
Our aides speak a variety of languages, including Spanish, Russian and Chinese. We will do our best to accommodate your loved one by locating an aide who speaks their language.

Why is there a four-hour minimum, if I only need a home health aide for two hours?
This minimum exists because we want to provide your loved one with the best possible care and believe that this time will allow for that type of service.

Why does a PRI only last for 90 days?
A patient's health can change quickly. In order to provide the best evaluation possible of your loved one's health this window of time has been established.

What is the difference between a licensed agency and a certified agency?
The regulations for licensed and certified agencies are similar. A certified agency is able to provide services that are reimbursed by Medicare and Medicaid. A licensed agency is not able to receive reimbursement from Medicare or Medicaid. Both licensed and certified agencies must comply with Department of Health regulations.

If I have an aide under Medicare for a certain number of hours and days a week and I feel that I need more than that, what can I do? Can I keep the same aide?
If a patient wishes to extend their hours, they can call the intake department and complete an intake to privately pay for the additional hours. Once the necessary information is provided, a Partners in Care' nurse will conduct an assessment of the patient. The HHA that is servicing the case will be asked if they can increase their service hours to accommodate the patient's need for continuity of care.

Are the home health aides screened and in what manner?
All candidates are screened in the following areas:
- Criminal Background Check - Background checks are completed to screen for felonies, misdemeanors, violations, outstanding warrants, and criminal time served. Partners goes as far back as possible.
- Social Security Check - This involves all counties the candidate has lived in or worked in
- Personal References
- Work References
- Employee Health Check - Candidates undergo a drug screening, physical and immunization update
- Proof of Address
- I - 9 Verification
- Certificate Verification
- Interview, skills assessment, entry level tests, including reading and writing
A candidate must pass all of the above criteria and as well as complete our training program.

Are Partners in Care Home Health Aides insured?
All Partners in Care Home Health Aides are insured and certified. They are not bonded.

How far in advance do I have to call to set up services?
We work to handle all service requests as needed from same day to several weeks in advance. It is helpful if we are given at least 24-hours advance notice of service need.

Once I call and provide all the necessary information to start services, what is the next step?
Once your call is completed, a Home Health Aide supervisor will be assigned to your case. The supervisor will assign a Home Health Aide to your case. The supervisor will then contact you to provide the name of the Home Health Aide and answer any questions you may have.

If I need to get in touch with someone at the office over the weekend/ holiday/night regarding an issue, will someone be available to help me?
Yes, someone is available in the office seven days a week. We also have staff on-call during non-office hours to assist you with any urgent service issues.

Do you accept any insurance?
We will work with you to determine what your benefit coverage is and what is needed in order for service to be reimbursed.
