Information about Home Safety Assessments

Are you concerned that an elderly loved one will trip and fall at home? Does your mom or dad have trouble maintaining balance or walking, or has he or she fallen lately? 

Partners in Care can arrange for a nurse to visit your loved one's home and perform a Home Safety Assessment. This low-cost assessment allows our trained nurses to see the home environment and your loved one's ability to get around, so we can provide recommendations for making the home more secure, which may help to prevent trips and falls and rehospitalization.

Partners in Care will also perform a functional assessment of your loved one to determine whether home care can help him or her live better at home. Home care services may include:

  • Companionship/conversation
  • A break for the primary caregiver (respite care)
  • Arranging appointments
  • Assistance with walking
  • Light housekeeping
  • Physical therapy
  • Skilled nursing
  • Ambulatory escort (i.e., taking the client to and from medical appointments)
