A Nurse or an Aide?

You want the best care possible, but you may be confused about whether you need a nurse or a home health aide. When you call Partners in Care, a member of our knowledgeable client services team will talk to you about your specific situation and will recommend the services that are right for you. If your needs are fairly straightforward or if you need immediate care, a clinical manager will review your profile before your care begins. If you are not sure what care is needed after speaking with our staff, we will send a nurse who will assess your medical and emotional needs and behavioral issues, and will evaluate the safety of your home. The nurse will make recommendations that address current and future safety and health care needs, as well as whether skilled nursing care or companionship is more appropriate.

Do you need skilled nursing services?

Our highly skilled nurses deliver a wide range of services, including:

  • Administering medications and treatments, including pre-pouring medications so they can be taken according to schedule
  • Immunization services, including flu shots and fertility injections
  • Round-the-clock nursing care following an operation, whether you are in the hospital (with approval from the hospital administration), in a skilled nursing facility, or at home
  • Skilled assessments, such as home safety and nursing home eligibility (including PRI Assessments)
  • Other nursing assessments

Will a certified home health aide meet your needs?

Our aides are assigned for at least four hours a day, with no minimum of days, and are available to fit into your schedule. Service can be short- or long-term, periodic or continuous. Our home health aides can help with:

  • Reminders to take medications
  • Recording of vital signs, including pulse and blood pressure
  • Getting to and from medical and other appointments
  • Food shopping, cooking, and meal preparation
  • Bathing, dressing, and grooming (bath service is also available separately)
  • Short-term personal assistance after injuries or procedures
  • Long-term care for people with Alzheimer’s disease and other debilitating conditions

No matter which services you choose, a Partners in Care registered nurse will oversee your Plan of Care—including supervising all staff and scheduling all services. The nurse supervisor will also work with your doctors, community support organizations, and other family members.
