PRI/Screen

Partners in Care nurses are trained to assess a client’s medical condition and determine whether he or she can currently function safely in the community or would be more comfortable in a skilled nursing facility.

Once you decide on a nursing facility, a Patient Review Instrument, or PRI, is required by law. Appointments for a PRI screening are available seven days a week and can often be handled within 24 hours of a request. Our nurses travel to all five boroughs of New York City as well as Nassau, Suffolk, Westchester and Rockland Counties.

A PRI assessment can determine:

• Medical condition/diagnosis
• Medications needed
• Special diets
• Therapies needed
• Physical and mental abilities and limitations
• Ability to perform daily tasks
• Behaviors that may need management, such as aggressiveness, anxiety, or depression

Partners in Care offers a comprehensive set of private-pay home-based health care services for people who are able to live in their own homes with professional assistance, as well as services for clients in nursing homes who need or want more personal attention.
